Psyllium husk

The verdict

A quiet yes — and the rare supplement that earns its place three ways at once. Psyllium is viscous soluble fiber: it lowers LDL and ApoB (modest but real, and the basis of an FDA-recognized heart-disease claim), it blunts the post-meal glucose spike, and it relieves the constipation that GLP-1 drugs reliably cause. Cheap, food-like, low risk.

It is an adjunct, not a drug. A ~5–10% LDL drop is worth having but won't carry an elevated ApoB on its own — that still belongs to diet, weight loss, and possibly a statin. Take it with plenty of water and space it from oral meds.

The claims, sorted

Psyllium is sold for gut, heart, and blood sugar. Unusually for a supplement, all three claims have decent backing — but they're not equally strong, so separate them.

One scoop, three levers

The reason psyllium does several jobs is one mechanism, not three. In water it forms a thick gel. That gel traps bile acids and carries them out in stool; the liver, now short on bile, pulls cholesterol from the blood to make more — and LDL/ApoB drop. The same viscosity slows gastric emptying and sugar absorption, flattening the post-meal glucose curve. And the gel holds water in the stool, which softens a hard stool and firms a loose one — the rare laxative that works in both directions.

This is why the form matters. The gel is the active ingredient. A fiber capsule with too little husk, or fiber that's been ground fine and baked, loses much of the viscosity — so it loses much of the effect. Stir the husk into water and drink it before it sets.

How big is the LDL effect — and is it enough?

Pooled RCT evidence (a large meta-analysis in the American Journal of Clinical Nutrition, 2018) puts psyllium at roughly 5–10% lower LDL, dose-dependent, around ~10 g/day. That's the basis of the FDA's authorized health claim that ~7 g/day of soluble fiber from psyllium may reduce heart-disease risk. ApoB (the particle count that tracks risk better than LDL-C) tends to move in step.

Set expectations honestly. A high-potency statin lowers LDL ~40–55%; psyllium delivers a fraction of that. So it's a genuine but small lever — a useful add-on to diet and, if indicated, medication. It is not a statin substitute, and an ApoB flagged "watch" or "high" is a diet-and-possibly-drug conversation, not a fiber-scoop conversation. Treat the LDL drop as a free side benefit of a fiber you'd take for the gut and glucose anyway.

Using it without the downsides

Three practical rules cover almost all of the risk. Water: psyllium swells — take it with a full glass and stay hydrated, or it can do the opposite of what you want. Start low: begin around ~5 g and build up; jumping straight to a big dose buys bloating and gas. Space it from pills: the same gel that traps bile acids can blunt absorption of oral drugs, so take psyllium ~1–2 hours apart from medications. Injected drugs aren't affected.

Phase 1 · Aggressive cut (Zepbound)

High value, and the phase where psyllium does the most. GLP-1 drugs slow the gut and constipation is one of the most common complaints — psyllium is a clean first-line fix. Meanwhile the LDL/ApoB and glucose-blunting effects line up exactly with a metabolic clean-up, and the satiety nudge is mildly helpful on a cut. Cheap, food-like, reversible: an easy n-of-1. Just keep the water up and don't expect it to move the scale by itself.

Phase 2 · Building muscle

Keep it, but for lipids and glycemia, not physique. It does nothing for muscle and isn't a protein source. A large dose right around a meal can dent how much you absorb, so on a high-intake build you may want to take it away from your biggest protein meals.

Phase 3 · Maintenance

Reasonable to keep indefinitely. The heart and blood-sugar benefits are exactly the kind of small, durable lever that pays off over decades. Low stakes, low cost — continue if it fits the routine.

For you

This one maps onto your panel almost too neatly. Three of your live issues — ApoB 89 ("watch"), insulin resistance / glycemic load (HOMA-IR 3.65, TCF7L2 T/C), and Zepbound-driven slow gut — are all things a single ~10 g scoop of psyllium nudges in the right direction. That's an unusually good ratio of cost to coverage.

My call — add it; it's the cheapest lever you're not pulling

Start ~5 g in water once daily and build toward ~10 g. Best timing is before your largest carb-containing meal — that's where the glucose-blunting matters most for someone with HOMA-IR 3.65 and a TCF7L2 risk allele. The ApoB nudge (~5–10% on LDL) is a welcome bonus, but be clear-eyed: at 89, "watch," your ApoB is driven mostly by diet, visceral fat, and weight, and your protocol — the ongoing loss and lipid-aware eating — is the real lever. Psyllium rides alongside; it doesn't replace a statin conversation if your number doesn't come down with the weight.

Two stack details specific to you. Lisinopril is oral, so separate psyllium from it by ~1–2 hours; your tirzepatide is injected, so it's unaffected. And don't take a big scoop right on top of your AM protein shake (whey + collagen + creatine + HMB) — the gel can blunt absorption, and you want that 170 g floor landing intact. Easiest: psyllium at a different meal than the shake. If Zepbound constipation is the main driver, that benefit shows up within days; the lipid/glucose effects you'll only see at your next panel.
