The daily multivitamin

The verdict

For a well-fed, well-tracked adult, the daily multivitamin is mostly noise — "insurance" you probably don't need. In nourished people, the large randomized trials show little to no hard-outcome benefit (no clear drop in death, heart disease, or cancer). The famous cognition signal is real but small and unsettled. Harm is low, but so is benefit.

The smarter move is targeted repletion: test for a specific gap, then dose that nutrient. A shotgun of two-dozen low-dose ingredients rarely fixes a real deficiency and rarely matters when you're already replete. One safety footnote up front: don't take iron without labs proving you need it.

A multivitamin contains 20-odd nutrients at modest doses. If you're already eating enough of most of them, what would one more small daily dose actually change — and if you're genuinely short on one nutrient, is a pinch of it inside a 20-ingredient pill the right way to fix that? Decide before reading on.

What the big trials actually found

This is the part the supplement aisle skips. When researchers ran large randomized trials in nourished adults and counted hard outcomes — heart attacks, cancers, deaths — the daily multivitamin mostly came back flat. The U.S. Preventive Services Task Force reviewed this body of evidence in 2022 and concluded the evidence is insufficient that multivitamins prevent cardiovascular disease or cancer in healthy adults. Physicians' Health Study II — a long randomized trial in male physicians — found at most a modest cancer signal and no all-cause mortality benefit. Pooled across trials, the honest summary is "not a hard-outcome lever in well-fed people."

Why the shotgun usually misses

Two reasons the "broad coverage" logic breaks down. First, if you're already replete in a nutrient, a little extra does essentially nothing — you can't bank most water-soluble vitamins, and topping up a full tank changes no outcome. Second, if you have a genuine deficiency, the modest amount inside a 20-ingredient pill is often too small to correct it; a real shortfall needs a real, dosed repletion of that one nutrient. So the multivitamin tends to be redundant where you don't need it and inadequate where you do — missing in both directions.

There's also a "first, do no harm" wrinkle. The USPSTF specifically recommended against β-carotene (it raised lung-cancer risk in smokers and people exposed to asbestos in trials) and against vitamin E (no benefit). Many multivitamins still carry both. Not every ingredient is neutral, so "it can't hurt" isn't automatically true.

The cognition signal — real, small, not settled

The strongest recent point in the multivitamin's favor is COSMOS-Mind (published ~2023), an arm of the COSMOS trial in older adults that reported a small slowing of cognitive decline over a few years versus placebo. It's a legitimate randomized finding and worth knowing. But three cautions keep it from settling the question: the effect was modest and measured on cognitive-test scores (a surrogate, not "stayed independent longer"); it hasn't been broadly replicated and remains debated; and it was studied in adults typically 60–70+. If you're in your early 40s, you're not the population it was tested in — so it's a thin reason to start, not a strong one.

The iron exception worth its own line

One genuinely important safety point. Men and non-menstruating people should not supplement iron without labs showing deficiency. Unlike most minerals, the body has no good way to excrete excess iron, it's pro-oxidant in surplus, and chronic overload is not benign. Many "complete" multivitamins include iron by default. If you ever take one without a documented low ferritin/iron panel, choose an iron-free formula. Iron is the cleanest example of why "broad coverage" can quietly be the wrong default.

By phase

Unusually for this desk, the multivitamin call barely moves by phase — it's "skip / optional" throughout. The only real nuance is at the bottom of a very low-calorie stretch, where genuine gaps become more plausible and worth testing for, not blindly papering over. Phase 1 · Aggressive cut (Zepbound)

Skip the generic MVI; keep testing. The one phase-specific risk is that very low intake on a deep cut can create real shortfalls (think iron, B12, vitamin D depending on diet). The fix isn't a shotgun — it's checking the few decision-relevant markers and repleting whatever's actually low. If you want a cheap backstop on a hard stretch, an iron-free multivitamin is a reasonable belt-and-suspenders, not a lever.

Phase 2 · Building muscle

Optional, low stakes. Calories and food variety are higher in a build, so gaps are less likely, not more. Protein, training, and creatine do the work; a multivitamin adds nothing to muscle. Skip unless a test says otherwise.

Phase 3 · Maintenance

Optional. Stable weight and a varied diet make the "insurance" case weakest. The COSMOS cognition angle only becomes worth weighing decades from now, in older age — and even then it's small. For now, keep the budget on testing the markers that change decisions.

For you

You're close to the ideal case against a blanket multivitamin: a sophisticated, well-fed adult who already tests and targets. You don't need a shotgun because you're already firing aimed shots — vitamin D measured (59, comfortably in range), creatine and protein running, omega-3 in range, psyllium on board. A generic 20-ingredient pill would be largely redundant on top of that, and "insurance" against shortfalls you've already checked for and don't have.

My call — default skip; you already do the better version

Skip the daily multivitamin. Your whole approach is the upgrade over it: you measure the few markers that matter and fix the specific gaps, which beats spraying low doses at problems you don't have. Adding a generic MVI on top would mostly be noise — and if it carried iron or β-carotene it could be marginally worse than nothing for a man who isn't iron-deficient.

The one situation to reconsider: a deep, low-calorie stretch of the Cut where intake variety drops. Even then, the right move is to test (iron/ferritin, B12, D) and replete what's low — see the vitamin D3 + K2 card for exactly that "measure a specific nutrient, then dose it" pattern. If you want a no-thinking backstop during that window, take an iron-free multivitamin and treat it as cheap insurance, not a lever you'll get evidence for.

Better spend of attention: keep retesting the markers that actually drive decisions — ApoB 89 (your live lipid target), HOMA-IR 3.65 (insulin resistance, the one I'd watch most), and vitamin D — and fix gaps one at a time. Food first where you can (see grandma's wisdom); a pill only for a tested shortfall.

Profile used: 41, South Asian male, 5′9″, Phase 1 cut on Zepbound, ~170 g/day protein. Already testing & targeting — vitamin D 59 (in range), creatine + protein + omega-3 (in range) + psyllium on board; ferritin/iron not flagged.
